Mise en place d'un protocole d'information sur la vaccination des parents contre la coqueluche en maternité : étude prospective réalisée au CHU d'Angers

Abstract : BACKGROUND. Pertussis remains a major health problem among infants. For a few years, a resurgence of pertussis has been documented and it has been noted that parents are the main source of infection for infants. This is why, since 2004, Health Authorities advise a familial vaccination called cocooning in the infant's entourage. We have evaluated the impact of information protocol about pertussis on parents in maternity.

METHODS. During a period of 3 months, during the stay in the maternity, parents have been orally informed and by a written document about the gravity of pertussis and the importance of a booster dose carried out as soon as possible after the delivery. Two months after the birth, mothers, who accepted to participate to the study, were inquired about the received information and the parental vaccination.

RESULTS. From January to March 2009, there were 1012 deliveries and 807 consents were collected. Seventy five percent of women, who signed the consent, responded to the questionnaire. The information about pertussis was given in 99,1 % of the cases, mainly by midwives (47,9 %), orally and by a document and it was considered pertinent by more 95 % of mothers. Thirty one percent of women received the information about pertussis either during or before the pregnancy. Sixty nine percent of mothers, who received a prescription of dTpa vaccine at the maternity, were vaccinated and 62,5 % of fathers. In one hand, vaccination was carried out by the consulting physician for 95,9 % of women and 94,1 % of men ; in the other hand, it was carried out during the first month after the delivery (81,4 % of mothers and 84 % of fathers).

CONCLUSION. The information protocol about the recommendations of pertussis vaccination is a success among parents at the hospital. It contributes to increase the level of immunization against pertussis of adults.
