L'insulinothérapie chez les diabétiques de type deux

Abstract : The term diabetes, which represents a major problem of public health, indicates in a general way "a metabolic group of diseases characterized by a hyperglycemia resulting from defects of the secretion or from the action of the insulin, or by both freshwater algae ". This hyperglycemia contributes in the long term to the appearance of vascular complications which represent the main part of the gravity of this pathology. Besides, the therapeutic objectives to achieve are rather strict and to prevent the complications thanks to a good metabolic control, with a multifactorial coverage of the pathologies associated with the diabetes (HTA, dyslipidemia) can strengthen the efficiency of the treatment. Our work had for objective to study the implementation of the insulin therapy in patients with diabetes type 2. At the moment there is no consensus on modalities of insulin introduction and it remains a subject of debate. However, insulin schemes can be prefer. On the other hand, those schemes are estimated according to needs insulin individual. Besides, the institution of insulin will have to take into account barriers which prevent from reaching and maintaining those objectives. So, the current trend is to begin insulin therapy with basal insulin in an injection in the bedtime, or with premix insulin in two injections per day. Then, when those schemes fail multiple injections can be envisaged. Besides, it's better to keep OHA, because the association with Insulin presents a considerable interest in the achievement of the glycaemic objectives, but also in the reduction of doses of insulin and its side effects. Current studies estimate the place of the association of incretines with the insulin, because their physiological effects could have an interest in the insulin therapy at the diabetics of type 2. Finally, in wait for new researches supporting the modalities of the insulin therapy quoted in this thesis, allowing to establish consensus as for the place of the insulin therapy at the diabetic of type 2, but also in wait of new associations and alternative ways with the aim of returning the insulin therapy more acceptable and more effective, it seems essential to prevent, to educate and to prepare the patients from the diagnosis of the disease on the importance and the role of a premature implementation of the insulin.
