Comparaison de deux méthodes d'anesthésie pendant le travail : la péri-rachianesthésie combinée et l'anesthésie péridurale. Étude prospective sur 35 dossiers

Abstract : Unlike the epidural analgesia (EA), the combined spinal-epidural analgesia (CSE) hasn't been studied enough to be able to draw real conclusions on its consequences on the mother, the son and the labor. This thesis aims at finding if the EA and the CSE are compatible on this point. We have realise the prospective study at two centres. The maternity of Port-Royal and Neker, from may 2010 to january 2011. We set up a survey in the delivery room, and we have gathered data from the obstetrical folder. Thirty five records were collected. The results of the study and the analysis of the literature don't highlight any significative difference between the two groups regarding the repercussions on the mother the child and work. On the other hand the results showed a rather faster relief of pain with the EA based on smaller morphemic dosses and local anaesthetic ones. This dissertation allows us to assert that the EA and the CSE are two comparable methods. Regarding the repercussions on work development, on the neonative part and the side effects. It showed that a midwife can take care of a patient under EA and CSE.
