Précarité et risque cardiovasculaire‎. Étude prospective dans trois centres de santé à Grenoble

Abstract : Objectives : The prevalence of health issues in people living in difficult socioeconomic conditions is increasing, particularly as concerns cardiovascular diseases. Cardiovascular pathologies are the second leading cause of death in France. The main aim of this study was to examine the potential relationship between precarity and risk of cardiovascular mortality in patients in primary prevention. Methods : A cross-sectional prevalence study, both descriptive and prospective, was conducted in three health care centers in Grenoble on 222 patients aged 40 years and older. Precarity was assessed using the EPICES score, a multidimensional index of deprivation. The ten-year risk of cardiovascular mortality was estimated using the SCORE scale in subjects without previous history of cardiovascular disease. Results : After taking into account age, sex and confounding factors, precarity was not significantly related to cardiovascular mortality risk except for the group of patients older than 65 years in whom the cardiovascular risk increased with precarity, due to a higher prevalence of diabetes. Cardiovascular risk factors like tobacco use, high blood pressure, diabetes, LDL-cholesterol or metabolic syndrome were not significantly higher or more frequent among the precarious population. Obesity and sedentarity rates were higher. Conclusion : The mission and objectives of the Grenoble health care centers have probably contributed to reducing healthcare disparities across socioeconomic populations. The study emphasizes the need to carry out public health actions focused on socially disfavored populations to improve prevention and management of cardiovascular risk in these patients.
