Difficultés de prise en charge par le médecin généraliste des personnes âgées en situation médicale et psycho-sociale complexe‎ : état des lieux à propos d'une enquête à Grenoble, Annecy et Roanne

Abstract : Objective We assessed the difficulties encountered by general practitioners in the care of the elderly with complex conditions, with a view to setting-up mobile community based geriatric units. Materiels The survey took the form of a telephone interview and a multimodal questionnaire of general practitioners in the geographic areas concerned. Results 65% of the GPs contacted by telephone said the survey interested them, but only 111 (32.8%) physicians replied to the questionnaire. The survey confirmed that there are specific features related to the care of the elderly, in particular multiple pathologies (41 replies). The first priority for intervention of the mobile geriatric unit should be the management of the elderly with cognitive disorders and behavioural psychologies (43 rep) that pose the most problems for physicians. The major expectation of respondents was that geriatric assessments should be conducted at home (28 rep). For the 3 cities, 95 to 160 patients might benefit from an intervention per month. The request for intervention should be made by the GP (69 rep, 72.6%), by telephone (95 rep, 86%) and the visit made within the following 48 - 72 hours (60 rep, 67.4%). Each city has its own specific requirements : respondents make fewer home visits in Annecy, request fewer geriatric consultations in Grenoble and more frequently make home visits, but less often work with paramedics in Roanne. Discussion Our results confirm the existence of difficulties in the care of elderly people with complex conditions and highlight that in order to fulfil their missions, the community mobile geriatric teams must adapt to the specific characteristics of each agglomeration.
