Influence de la durée objective du sommeil sur l'équilibre glycémique chez les sujets diabétiques de type 1‎ : étude DIAPASOM 2

Abstract : OBJECTIVES : - Exploring the link between objective sleep duration (actimetry of 3 consecutive nights) and glycemic control (HbA1c) in type 1 diabetes, and - Study the relationship between the average duration of sleep and hypertension (Ambulatory Blood Pression Monitoring), sleep quality (score of Pittsburgh) and the risk of obstructive sleep apnea syndrom (Berlin questionnaire). MATERIAL AND METHODS : prospective study, single center (CHU Grenoble), from December 2009 to February 2011, 55 patients with type 1 diabetes (interim analysis). RESULTS : Non-significant trend to a higher HbA1c if the mean and median duration of sleep in 3 nights are shorter. Splitting the population into tertiles showed a significant U-shaped relationship (ANOVA 0,018) : mean HbA1c = 8,54% for sleep duration from 4,98 to 6,4 hours (first tertile), and equals to 7,58% for sleep duration from 6,4 to 7,24 hours (second tertile), p-val < 0,017. HbA1c of the third tertile (7,24 to 9,28 hours) equals to 7,81%. Logistic regression showed a significant influence of Pittsburgh's score (OR = 4 for HbA1c > 7% if Pittsburgh > 5, p-val = 0,046) and BMI (OR = 0,209 for HbA1c > 7% if BMI > 23,68 kg/m², p-val = 0,047). The average duration of sleep was significantly shorter : - within patients with non-dippers compared to dippers patients (6,48 hours vs 7,22, p = 0,028), - within patients with a high-risk of obstructive sleep apnea syndrom, with a difference at the limit of significance (6,55 hours vs 7,06, p = 0,051). CONCLUSION : a shortened sleep duration is associated with increased HbA1c (to be confirmed with the final study of 140 patients).
