Chirurgie aortique mini-invasive‎ : intérêt de la vidéo assistance ?

Abstract : PURPOSE: To compare results of video assisted mini laparotomy (VAML) versus non assisted mini laparotomy (ML). METHODS: Between 2005 and 2007, 88 patients underwent aortic surgery for abdominal aortic aneurysm (AAA=50) or aorto iliac occlusive disease (AIOD=38). Surgical techniques were VAML (n=40, 18 AOID and 22 AAA) or ML (n=48, 20 AOID and 28 AAA). RESULTS: Regarding AIOD: Mean operation time is 205 min (VAML) vs 119 min (ML) (p<.05), and mean aortic clamping time is 32 min vs 30 min (p=NS). In both groups, the median length of stay after surgery is 6 days (p=NS), morbidity rate is 30% (p=NS) and nobody died. The mean follow-up time is 11,4 vs 10,1 months with a late morbidity rate at 22 vs 20% (p=NS). Regarding AAA: Mean operation time is 188 min (VAML) vs 140 min (ML) (p<.05), and mean aortic clamping time is 49 min vs 47 min (p=NS). During the hospitalisation transfusion was necessary in 22 vs 10,7% (p=NS). Solid alimentation was possible at 2,5 vs 4 days (p<.05) and déambulation at 3 vs 4 days (p<.05). In both groups, the median length of stay after surgery is 6 days (p=NS). Morbidity rate is 26,2 vs 32% (p=NS), one patient died in each group. Mean followup time is 12 vs 9,5 months with a morbidity rate of 27 vs 14% (p=NS). CONCLUSION: Video assistance doesn't bring the hoped advantages concerning duration of the operation and hospitalisation length of stay. Its interest could grow with the development of new automatic suture devices.
