Évaluation de la capacité cognitive des personnes âgées à gérer leur médicaments‎ : données préliminaires du dépistage par le test dit "du pilulier"‎

Abstract : Drug-related hospital admissions of older persons attributable to nonadherence range from 8 to 11%. The purpose of this prospective study is to assess objectively medication management skills in the elderly, and to document the association between presence of cognitive impairment and difficulties in taking medication independently. Subjects were recruited among general practitioners' patients, when refilling prescriptions. Inclusion criteria required that subjects be aged 75 years and older, and take at least 3 chronic medications. Patients receiving assistance to manage medications were excluded. Cognitive assessment used the MMSE, the Trail Making Test Parts A and B, clock-drawing test and verbal fluency. In order to assess the ability to manage medications, patients had to fill a 7-day pillbox with their own medications. This was called the "pillbox test". The main outcome measure was the percentage of correctly loaded pills. Twenty-four patients completed the study. The mean pillbox test score was 93.7%. Three subjects scored below 80%. The pillbox score was inversely correlated to Trail Making B (r = - 0.63 ; p = 0.001) and Trail Making A (r = - 0.54 ; p = 0.007). Finally, we identified in this study a need for medication assistance in 12.5% of polymedicated elderly subjects that considered themselves able to self-manage medications. As a cognitive factor, executive function is strongly associated with the ability of patients to fill a pillbox.
