Arthroplastie par implant MiNiMa 1ère génération® dans l'arthrose des articulations inter-phalangiennes distale et proximale des doigts longs‎ : étude rétrospective de 28 cas avec un recul moyen de 3,5 ans

Abstract : MiNiMa first generation implant is a unconstrained pyrocarbon prosthesis, that acts like a spacer. The aim of this study is to evaluate the long term outcome of 21 patients with 28 arthroplasties. Patients were assessed with subjective and objective datas like pain, range of motion, instability, bone remodeling and satisfaction. At the average follow-up of 3.5 years, only 14 of 28 implants are still in place. At the latest follow-up, the postoperative range of motion of the interphalangeal joints goes from 0° to 70°, with an average of 24°. Pain is absent or occasional in 78% of the patients with the implant still in place. 36% of the patients are satisfied and 27% completely satisfied. The satisfaction is unaffected by the joint involved in the surgery: for the proximal interphalangeal joint, the average is 2,9/5 and for the distal interphalangeal joint, the average is 3,75/5. The pinch strength has recovered to 56% of controlateral pinch strength. The interface bone-pyrocarbon presents a good bone remodeling in 61% of the arthroplasties. The major complication of this type of arthroplasty is lateral luxation of implant. Frontal instability due to inability of collateral ligaments is present in 40% of the joints and represents 86% of the causes of surgical revision. 50% of the arthroplasties require surgical revisions. Failures occur preferentially in the first six months after the surgery and concern mostly radial digits. These results are poor compared to literature: lesser range of motion and higher surgical revision rate. This arthroplasty does not hamper an arthrodesis or another arthroplasty.
