Fièvre des polymères après intoxication au fart fluoré‎ : un rapport de trois cas avec revue de la littérature

Abstract : Three snowboard instructors were received in Emergencies of Hôpital du Pays du Mont Blanc for dyspnea and influenza-like syndrom after using a fluorinated wax. Given the history, diagnosis of polymer fume fever was made before the clinical and paraclinical findings. Their condition was self-limiting within 24 hours after appropriate treatment. Polymer fume fever is known since 1950, it is due to pyrolysis by-products of fluorocarbon compounds from 650 ° C. It is represented by dyspnea and influenza-like symptoms. It can lead to APE, ARDS, or death. In the long term, cases of pulmonary fibrosis have been described. Its treatment relies mainly on prevention. Smoking in presence of fluorocarbon compounds is a major factor in causing the disease because the temperature can reach up to 850 ° C; its involvement is widely recognized in the field of occupational medicine. For 20 years, fluorinated waxes used in the middle of skiing have been responsible for serious cases and led to the realization of clinical studies in scandinavian countries and in United States. Their environemental impact is still unclear. The frequency of this pathology is underestimated by default of diagnosis, it threatens especially professional skiwaxers in the field of competition, however it is not recognized as an occupational disease. The developement of economic competition promotes the access of these products to individuals and increases the risk of accidental poisoning. Preventive measures are stopping the use of fluorinated waxes, or if failing, wearing personal protective equipment, the existence of an adequate system for evacuating smoke and avoid smoking when in presence of fluorinated waxes.
