Intérêt de l'activité physique aérobie et du renforcement musculaire dans la polyarthrite rhumatoïde : analyses systématiques de la littérature et méta-analyses d'essais randomisés contrôlés

Abstract : Objective: The objective was to evaluate the efficacy and the safety of aerobic and resistance exercises in Rheumatoid Arthritis (RA) patients. Methods: A systematic literature search of randomised controlled trials (RCT) was performed using Medline, Embase and Cochrane databases. Data were pooled using the inverse of variance model. Results: 14 RCT, including 1040 patients, comparing aerobic exercise and usual care, were pooled in the meta-analysis. Aerobic exercise significantly improved quality of life, function, pain and radiographic damage. Supervised exercises seemed to achieve better outcomes. 10 RCT, including 547 patients, comparing resistance exercises and usual care were pooled in the meta-analysis. Resistance exercises significantly improved muscle strength function and walk test. Subgroup analysis revealed a trend toward higher efficacy associated with high intensity. Withdrawals and adverse events were well-balanced in both intervention groups and control group. Conclusion: Aerobic and resistance exercises are efficacious and safe in RA. However, the methodological quality of clinical studies on this field needs to be improved.
