Étude comparative de la restitution immunitaire sous traitement antirétroviral de première ligne entre un pays du Nord et un pays du Sud

Abstract : The study aims to highlight potential disparities between North and South immune restoration in patients HIV1 infected, by first line antiretroviral treatment at least two years. It is a retrospective comparative study of two cohorts of 200 patients randomly selected from the active files of people living with HIV in Ouagadougou / Burkina Faso and Chambery / France. The descriptive study of socio-economic data collected shows a majority of women followed in the South (64%) with severe immunosuppression at start of treatment. Treatment success, defined here as a CD4 above 500/mm ³ after two years of treatment, is higher in North (41% versus 15% in the South), but the slope of recovery of CD4 is parallel in both cohorts , with an average initial CD4 count to 220 in Chambery and 136/mm³ in Ouagadougou. The high number of lost to follow up (36%) and the number of deaths in South undervalued represent a bias in this comparison. The success in the South, regardless of the antiretroviral therapy, depends on early detection for faster development on ART (in 2010, the WHO recommended to initiate ART with CD4 counts less than 350/mm ³ for all patients ), a revival of lost to follow up, a biological monitoring of quality and in particular a systematic measurement of viral load.
