Analyse qualitative des ateliers de formation à l'insulinothérapie fonctionnelle proposés au CHU de Grenoble

Abstract : Background: Functional insulin therapy aims to transfer to the patient the decisional process and the management of its insulin treatment, this according to the various acts of everyday life, and particularly meals. Training workshops in this practice have been implemented by the unit of Endocrinology Diabetology at the University Hospital of Grenoble. Objectives: The aim of the study is to assess the benefits to patients of this training, as well as their adherence to the method, and to think about to whom it can profit more. Method: This is a retrospective qualitative study in the form of semi guided interviews performed on 11 patients with type 1 diabetes, The patients were randomly selected and participated in the functional insulin therapy workshops between 2006 and 2008. Results: Patients report that the course was beneficial, in particularly for the basic knowledge, but probably too rich in information delivered at once. Moreover, some are not adhering to the method according to the criteria defined in the study (in particular the adaptation of insulin to carbohydrate content of meals). Others have difficulties in practicing. The reasons are multiple, sometimes associated with a too late learning art with a return to earlier practices. However, a number of common elements such as the acquisition of freedom in alimentation, a more comfortable life, a reduction of anxiety after the course are cited. Conclusion : If we want to continue these trainings without changing their form and content, we will probably have to better select the surveyed patients. The increased follow-up by the education staff after the course has a special significance.
