Évaluation d'un programme d'éducation thérapeutique des enfants asthmatiques au sein d'une école de l'asthme‎ : les jeux de l'air

Abstract : Background: Since fifteen years, the asthma prevalence has been increasing continuously in France and all over the world. Therapeutic education centers have been created in France, but few have been evaluated. Aim: The purpose of our study was to assess the impact of the Grenoble hospital therapeutic education program on asthmatic children, in order to improve its efficiency. Methods: Our study enrolled 6 to 15 years old children who attended the three Grenoble educations program's sessions, and may have been addressed by their practitioner, their pediatrician or the emergencies. Children were followed up during nine months; every three months, they were seen in consultation at the hospital and answered a questionnaire. The primary outcome was the modification of the baseline lung function tests at nine months of follow-up. The study took place from October, 2007 till June, 2009. Results: Twenty-for children were enrolled on the education program. Sixteen (67 %) were followed up during nine months. None was addressed by his general practitioner. Most of them (62 %) had moderate persistent asthma. There was no improvement of the baseline lung function tests after nine months of follow-up. We observed a significant improvement of the quality of life, the degrees of self-confidence for parents, and the use of the written asthma action plan. Positive results tended to fade after six months of follow-up. One of the groups consisted of less severe asthmatic children: their results seemed better than in the other groups. Conclusion: Our study demonstrates how the "Jeux de l'Air" improves quality of life, self-confidence and knowledge. It let us check that the program match the quality criteria of a patient's therapeutic education edited by the HAS. It would be useful to check our results on a larger population, and to evaluate the influence of the severity of asthma on the effectiveness of education programs. Involving more general practitioners in these education programs would increase the number of patients to be educated.
