Déterminants du lieu de consultation pour les soins primaires en gynécologie obstétrique‎ : enquête auprès de 521 femmes consultant au CHU de Grenoble

Abstract : Historically, the gynaecological and obstetric follow-up of the women was heavily insured by the gynaecologists. The decrease in number of doctors has changed women's choices. The study objective is to determine the reasons of the women guidance for their gynaecological primary care to the specialists practicing at the hospital. A declarative survey was undertaken through a questionnaire given to women who consult in the gynaecology-obstetrics unity of the University Hospital of Grenoble in March 2008. The women sent by a gynaecologist-obstetrician were excluded. 521 questionnaires (73.18% of visits) were returned, 347 were used: 265 women (76.36%) coming on their own and 82 women sent by a general practitioner or emergency doctor. The primary cares represent the majority of consultation reasons, especially for women coming on their own. General practitioners relay complex cases to specialists, mainly with regard to chronic diseases and fertility disorders (p <0.0001). 65 on 70 % of the women have notion of a general practitioner skill in this domain (p < 0, 0001) and rely on them. Women consulting on their own prefer gynaecologist because they think he is more competent and because they are accustomed. They may accept the general practitioner care, especially if they have already consulted in this area before (p <0.0001). It would be useful to analyze the consultations content to know whether women sent by general practitioners present pathologies more complex than those come from their own initiative. This study explains that the consultation key determinant for gynaecology and obstetrics primary care at the gynaecologist is the lack of information for patients on the general practitioner skills. Therefore, it seems urgent to implement an information campaign promoting the general practitioner skills for gynaecology and obstetrics primary care.
