Intérêt de l'élastographie ShearWave pour la prise en charge des nodules thyroïdiens : étude Swethy : analyse intermédiaire

Abstract : Objective: The purpose of this study was to evaluate ShearWave elastography (SWE) for thyroid nodules diagnosis. The main outcome is the number of FNAC avoided by a benignity criterion. Patients and methods: This work is an intermediate analysis of a clinical research protocol. 34 patients were included in a prospective trial. Data were collected 7 month after the study start. All patients were referred for fine needle aspiration cytology (FNAC) or thyroid nodule surgery. Each nodule was analysed at sonography to evaluate the ability of US individual features and TIRADS score to estimate benignity and malignity. Then elasticity index (EI, kPa) was measured in the nodule and in surrounding tissue (Supersonic Imagine, France). Correlation was done between US parameters, TIRADS score, elasticity, FNAC and histologic results. ROC curves were used to compare area under curve (AUC) of US, elastography and combined score. Results: No FNAC could have been avoided by TIRADS score. EI was higher in malignant (68kPa) than in benign nodules (41kPa). SWE is able to classify nodules with an AUC measured 0.8611, and to predict benignity with those performances (threshold 46kPa): Se 62.5%, Spe 100%, VPP 100%, VPN 25%. It would have avoided 15/24 FNAC for benign nodules. A combined score (TIRADS <5 and threshold <68kPa) can predict benignity with those performances: Se 95.83%, Spe 100%, VPP 100%, VPN 66.67%. It would have avoided 23/24 benign FNAC. AUC of ROC curve is measured 0.9792. Conclusion: ShearWave elastography could reduce the number of unnecessary FNAC or surgery of benign nodules. A clinical and ultrasonographic follow-up remains needed.
