Apports de l'épreuve d'effort chez l'homme de 40 ans ou plus, asymptomatique, poursuivant ou reprenant une activité physique et sportive

Abstract : Aims : To assess the prevalence of abnormalities during exercise testing and the incidence of cardiovascular events during follow-up. Methods : 556 asymptomatic athletes over 40 years old, without past coronary events, performed a graded maximal exercise test between January 2007 and December 2010. In December 2011, the cardiocirculatory events and coronary revascularization procedures were collected prospectively. Results : One hundred and eighty (32.8%) exercise tests were considered abnormal : 55 (30.6%) subjects had exaggerated systolic blood pressure response to exercise, 84 (46.7%) arrhythmia or a cardiac conduction problem and 71 (39.4%) ST-segment depression. The percentage of these abnormalities increased as subjects were older and had at least one cardiovascular risk factor, whereas physical fitness and training level did not increase the frequency of abnormal tests. In December 2011, during follow-up, only one subject had a major cardiovascular event and two subjects had a coronary revascularization procedure; no deaths were observed. Conclusion : These data suggest that exercise testing is relevant in asymptomatic sportsmen older than 40 years due to the high prevalence of abnormalities. The sensitivity of this test seems superior in subjects older than 50 years with at least one cardiovascular risk factor. This study cannot conclude on the relevance of exercise testing in predicting cardiovascular events due to their low occurrence in this population and a short follow-up period.
