Évaluation de la nécessité d'un bilan biologique exhaustif sur la prise en charge de la gastro-entérite aiguë virale de l'enfant nécessitant une réhydratation intra-veineuse

Abstract : INTRODUCTION: In presence of children with acute viral gastroenteritis requiring intravenous (IV) rehydration, in pediatric emergency rooms, prescribed biological tests are often exhaustive and highly variable, depending on each patient. Our study aimed at assessing whether measuring other elements than serum sodium and potassium, urea, alkaline reserve and glycaemia was relevant for the establishment of the treatment. METHOD: This prospective study was carried out in the pediatric emergency department in Bordeaux, France. We included children between one month and fifteen years of age, infected with viral acute gastroenteritis, whose biological tests had been performed and who had received IV rehydration. The physicians filled a form mentioning the biological criteria that had motivated a change in the IV perfusion, the orientation of the patient and the follow-up biological exam. RESULTS: We included 98 children. There were 19 modifications of IV rehydration, 18 of which resulted from pathological serum sodium and/or potassium levels. One resulted from high CRP. The analysis of the forms did not allow us to know whether a pathological biological result influenced the orientation of the patient. The modification of potassium supplementation and a pathological serum creatinine seem to be responsible for follow-up biological exams. Glycaemia had no impact. However, IV perfusions consistently contained glucose. CONCLUSION: Measures of serum sodium level, kalemia and alkaline reserve, as well as glycaemia in the absence of initial supplementation, are always required on the admission. The relevance of measures of creatinine and CRP remains unclear. Measures of other biological elements seem to have no impact.
