L'autonomie des sages-femmes : lecture sociologique de leur gestion des efforts expulsifs dans deux maternités franciliennes

Abstract : Midwives' autonomy: Sociological study about their management of expulsive efforts in two maternity around Paris France is the only country to have recommended limiting the duration of expulsive efforts 30 minutes before instrumental extraction. Studies showing no real indication, these guidelines are not applied in all maternities. Objective of this study is to bring a sociological perspective on the midwives' autonomy through this practice. Twenty interviews were conducted in two maternity around Paris (level 1 and 3) , completed with phase of participant observation. The aim is to show the similarities and differences in practices surrounding childbirth, making a focus on the management of expulsive efforts. Physiology's professional are interviewed about their feeling of autonomy, or their real autonomy, and what they want to change in their profession. Our study reveals that this autonomy is very different in the two teams studied, consequence of different interprofessional negotiations with obstetricians. Non-compliance with the rules or participation in protocols' development contributes significantly to the midwives' autonomy themselves responsible for the duration of expulsive efforts.
