Accouchement du siège et utérus uni-cicatriciel : évaluation du pronostic materno-fœtal

Abstract : Breech delivery and uterus with one scar: evaluation of maternal and neonatal prognosis Objective: The purpose of this study is to evaluate the maternal and neonatal outcomes in the event of breech presentation associated with uterus with one scar. Material and method: It is about a retrospective, comparative, bicentric case control study on maternities of Port-Royal and Saint-Vincent de Paul. 87 patients were included having uterus with one scar, a foetus in breech presentation and term higher than 35 weeks of gestation, paired with 87 witnesses having a healthy uterus. Results: Among 87 cases 13 were finally delivered vaginaly (15%) against 47% in the control group (p<0,001). The acceptance of the vaginal delivery is more important in the control group (68% vs 41%, p<0,001). The realization of a radiopelvimetry is higher in the case group (83% vs 67%, p=0,01). The rate of perineal damage is higher in the control group (39% vs 14%, p<0,01). Conclusion: We did not highlight increase in maternal and neonatal morbidity. It thus appears possible to us to suggest vaginal delivery to these women if none contraindication with this one exists.
