Intérêt de la mesure du pH au scalp dans la décision d'extraction fœtale : évaluation des pratiques professionnelles à la maternité de Port-Royal

Abstract : The role of fetal scalp blood pH in fetal extraction decision : Evaluation of professional practices at Port-Royal Maternity. Hospital Fetal monitoring during labor is commonly used to identify metabolic acidosis. Intrapartum fetal heart rate monitoring has a good sensitivity but a low specificity. Methods such as fetal scalp blood sampling are recommended to confirm whether fetal oxygenation is sufficient or not. Fetal scalp blood pH is the actual gold standard procedure. It is the one used in Port-Royal Maternity Hospital in case of fetal distress. We tried to evaluate how important fetal scalp blood pH is when it comes to take a decision about a fetal extraction compared to the single analysis of fetal heart rate monitoring. Two questionnaires including sixteen clinical cases were submitted a month apart to midwives, student midwives and doctors working at Port-Royal Maternity Hospital. Results showed that fetal scalp blood pH has lots of benefits such as reducing cesarean section rate or forceps deliveries. There is a better agreement regarding decisions based on fetal scalp blood pH analysis rather than fetal heart rate monitoring. There is a high interobserver variability in the interpretation of fetal heart rate monitoring.
