Analyse du contenu du carnet de santé présenté par les patients à l'accueil des urgences à Mayotte : efficience et plus-value du système mahorais

Abstract : Background: The patient's medical file is a key element to a proper functioning of an emergency department, but it is often missing. In Mayotte, a system of health record booklet is used as a complete medical file. This seems to be a very efficient way to share medical data between medical staff members. Objective: Determine the number of patients consulting at the emergency service with their mahoran health record booklet or any other medical paper file; assess the completeness and the relevance of the medical data written in the file as the ANAES recommends it; the third purpose is to evaluate the impact of these data on the care of the patients. Method Retrospective study of the medical files of every patient consulting at the emergency service of Mamoudzou over 72 hours through a questionnaire about the existence of the record and completeness of its content and about its interest for the care of the patient by the welcoming emergency nurse. Results: Among the 328 patients included in the study, 272 brought medical files with them to the emergency service (82,9%), and 252 files were mahoran health record booklets. The data written on the files corresponded to the ANAES recommendations of 2003. In 27,6% of the paper files, we found medical events witch could not be found on the computer system. Reading the paper file changed the decisions of the nurse in 37,9% of cases. Discussion: This study aims to show the interest of this tool in the context of missing information regarding the patient's medical past when they consult at the emergency department, although it only contains descriptive statistical. This health record booklet has every feature required to be a patient shared file.
