Contre-indications à l'altitude chez les sujets avec une maladie coronarienne ou un asthme : une revue de la littérature

Abstract : Background: More and more patients suffering from pre-existing conditions wish to spend some time at a high altitude and they are turning to the medical community to get their "safety pass". Purpose: Define clearly the contraindications related to an altitude of 2500m and above for patients who suffer from coronary heart disease or asthma. Methods: Review of the literature, inspired from the PRISMA methodology, searching the following data bases: Medline, Cismef, Cochrane Library, Google Scholar, SUDOC. The combination of the search terms: "altitude", "preexisting medical conditions", "coronary artery disease", "asthma" + limitation : "French or English" and "since 2003" has been used by Medline and is adapted to other data bases. The recommendations for the decision or not to allow a staying at a high altitude above 2500 m were synthesized, with the level of evidence. Results: 146 titles were found after the search in the data bases, and 21 articles were finally chosen and analyzed. The hand search have respectively produced 9 extra articles. Depending on their authors, the recommendations sometimes differ and have, on the whole, a low level of evidence, due to the lack of prospective studies devised to answer the given question. The pre-existing condition must be stable, low or moderately severe, in asymptomatic patients, who live at sea level, before the staying at a high altitude. Limitations: The main bias is that it was performed by only one researcher. Conclusion: based on this scientific literature, a consensus by experts using the Delphi methodology may be implemented in order to define new recommendations.
