Repérage de la fragilité en médecine générale avec le Gérontopôle Frailty Screening Tool : étude de prévalence et faisabilité en médecine générale

Abstract : Background: The Gérontopôle Frailty Screening Tool (GFST) is a tool developed for general practitioners (GP) to identify frail patients and designed to be used for persons aged 65 and more with no physical disability. Objectives: To evaluate the prevalence of frailty in community-dwelling persons aged 65 and more with the GFST. Methods: GP filled in the GFST for every 65-year-old and more patient who came for a consultation. The patient was identified as frail if he/she was considered frail by the GP after the initial questionnaire with frailty criteria. The inclusion register noticed the total number of patients aged 65 and more that consulted the GP during the study period. Results: Sixteen GP participated, 522 patients were included; an inclusion register was available at 11 GP so 379 patients from the inclusion register were included. The mean age was 76.1 years +/- 7.3 years, there were 51% of women. In the population from the inclusion register with no physical disability (n=372), 91 were identified as frail: 24.5% CI 95% [20.2%-29.2%]. 33.3% of 75-year-old and more patients (n=204) were identified as frail, IC 95% [27.0%-40.3%]. The inclusion rate was 67% to 93%, depending of GP, with a median of 83%. Conclusion: The GFST seems to be a good frailty screening tool in primary care: the prevalence of patients identified as frail is close to the reported prevalence in literature and it is easy to implement in the consultation. More studies are necessary to evaluate the interest of a systematic screening.
