L'autoprescription des antalgiques de palier II chez le médecin généraliste

Abstract : Introduction. Self prescription is a current practice in general practioners. Grade II analgesics are implied, and are at risk of addiction. The main objective was to describe the way general practioners were self prescribing grade II analgesics. The secondary objective was to determine whether their self prescription was influencing their grade II analgesic's prescription to patients. Methods. An epidemiologic and descriptive study was led through an on-line questionnaire which was sent to 6255 general practioners from Haute-Normandie, Rhône-Alpes, Languedoc-Roussillon and PACA between March and April 2014. PACA's results (n=3) were excluded from the analyses. Results. Among the 498 responses, 40,4% of the doctors had already prescribed themselves grade II analgesics. Most of them have a poor use. However, 27% of them were taking a pill 2 to 6 times a year, 8% more than 7 consecutive days, and 33% were notifying side effects. Factors associated with self prescription are: being a male (p=0.000), not having a physician other than oneself (p=0.076), chronic pains (p=0.000), psychiatric diseases (p=0.040), and polymedication. 86% think that their self prescription is objective, nevertheless only 73,2% think that their prescription to patients is similar to their own self prescription. Discussion. Grade II analgesic's self prescriptions are often reasonable, however the risk of misuse invite us to be cautious. It was difficult to conclude whether self prescription was influencing grade II analgesic's prescription to patients.
