Étude de l'application des recommandations de la société française de cardiologie concernant la réalisation d'un électrocardiogramme lors de la visite de non contre indication aux sports en compétition de 12 à 35 ans

Abstract : Since 2009, to reduce the incidence of sudden death during the effort of the young athlete, the French Society of cardiology recommends practice routine electrocardiogram during the consultation for medical certificate for all competitive athletes aged 12 to 35 years. These recommendations seem rarely applied in practice. The objective of this study is to evaluate the level of application of these recommendations. The secondary objective is to determine the factors influencing their application.A questionnaire was sent to General Practitioner of Haute-Savoie by internet, via a listing of the Council of the Medical Association. A second questionnaire, on paper, was filled by competitive athletes directly at sporting events or in their clubs. The sports were selected based on the epidemiology of the cardiology sudden death.. 45 doctors and 112 athletes completed the questionnaires. 27% of physicians applied the recommendations of the French Society of cardiology. 28.5% of the athletes had been detected as the recommendations. The only criteria significant influencing the recommendations were the formation of the general practitioner specialized in sport (p0.003) and the level of practice of sportsman (p0.016). The main obstacle is the lack of training of general practitioner. Other brakes are the lack of evidence on the effectiveness of screening, lack of time for general practitioner, cost, and lack of motivation of some athletes for such screening. The level of application of the recommendations is low, but increases compared to previous work. To allow more efficient screening of sudden death of young athlete, general practitioner should be better trained and sportsmen more aware. Given the low incidence of sudden death, further prospective large-scale studies to establish strong evidence of the effectiveness of screening is needed. They would probably permit a better acceptability of screening by general practitioner... and sportsmen.
