Étude de la sensibilité viscérale dans la maladie de Crohn en rémission et ses liens avec le tonus vagal et les variables psychologiques : comparaison avec le sujet sain

Abstract : BACKGROUND: Autonomic dysfunction and mood disorders are frequently described in Crohn’s disease (CD) and are known to influence visceral sensitivity. We addressed the link between vagal tone, negative affect and visceral sensitivity in CD patients. METHODS: Rectal distensions to a discomfort threshold of 70% and onset of pain were performed in CD patients in remission and healthy controls. Autonomic parameters were evaluated with heart rate variability and electrodermal reactivity. Psychological parameters were evaluated thanks to questionnaires. RESULTS: We showed that CD patients had i) higher tolerance to rectal distension pressures (43±3mmHg versus 30±2mmHg, p=0,002), ii) reduced vagal tone, iii) decreased sympathetic reactivity during an aversive stimulus, iiii) higher scores of depressive symptomatology (p=0,048), less self-esteem (p=0,048), and were more vigilant about their symptoms (p=0,02). On the other hand, CD patients’ strategies of coping were as good as healthy controls. CONCLUSION: Patients with quiescent CD, in the absence of IBS, are hyposensitive to experimental rectal distension. These data provide further evidence that anxiety and depressive symptomatology in addition to autonomic dysfunction modulate visceral pain perception in quiescent CD patients.
