Événements indésirables liés aux médicaments : terrain, caractéristiques, gravité, évitabilité : étude prospective dans un service de médecine

Abstract : The adverse drug events represent a problem of public health, of part the morbi-mortality and the financial and human additional cost which they engender. At present, the objective is to reduce the grave and avoidable events. Our study is a prospective study in a department of medicine during 3 months. The ground is the elderly person, the woman, the poly-pathology, the undernutrition, the poly-medication. The known medicinal interactions, a misuse, an intercurrente disease, an interference with a chronic disease are frequently present. Medicine with cardiovascular aim, in particular inhibitors of the enzyme of conversion, sartans, diuretics and anti-vitamin K is the first ones in cause. The evolution is most of the time favorable, but half of the cases is grave and avoidable. It demonstrates a link between age of more than 75 years and adverse drug event due to medicine with cardiovascular aim, between medicine with anti-inflammatory aim and gravity, between intercurrente disease and avoidability, between misuse and avoidability. The gravity is favored by a fragile ground, the presence of an intercurrente disease. The avoidability is favored by a fragile ground, a hospitalization. Medicine with cardiovascular and anti-inflammatory aim favors the gravity and the avoidability. The fight against the adverse drug event must be individual (prescriber/patient/prescription) and collective, with for objective the management of the risk oriented on the patient’s security. We realized a guide of drug safety surveillance.
