Quelles sont les perceptions du vaccin anti-HPV par les médecins généralistes et quelles sont leurs attitudes face à une patiente réticente ? Étude qualitative par entretien semi-dirigé auprès de médecins généralistes de Haute-Savoie

Abstract : The HPV infection is the most frequent genital infection, globally. The 16 and 18 HPV types are responsible for 70% of cervical cancers. There are 2 vaccines against HPV which are used for primary prevention. In France, general practitioners (GPs) are the main players in the vaccination process.The scope of this study is to collect GPs’ opinions on anti-HPV vaccines as well as understanding their behaviours when they are facing a reluctant patient; the goal being to increase the vaccination coverage which currently remains at around 30% in France.This is a qualitative study consisting of semi-structured interviews, performed with 18 GPs in Haute-Savoie.Most of the interviewed GPs are rather in favour of the anti-HPV vaccination. Some of them remain wary considering the lack of perspective for this vaccination.They specifically highlight their fear for secondary effects with, sometimes, an analogy made with the ones seen for the hepatitis B vaccine. This fear is amplified by the media. When facing a reluctant patient, most GPs usually explain once again the benefits of the vaccine. Others remain at the disposal of their patients, bring the topic back up later, reassure them, try to be understanding. Some GPs also want to scare patients and push them to detect the virus by the Pap smear test. Those who are not convinced do not insist.The anti-HPV vaccine is a recent one. GPs need more data on its efficacy and the morbid-mortality of the cervical cancer so they can be fully convinced and persuasive with their patients. One can also ask whether this vaccination could be proposed for boys in order to decrease the prevalence of oncogene HPV.
