Consultation préventive à domicile pour les patients âgés : étude prospective pour déterminer la valeur pronostique d’un outil d’évaluation des risques du domicile, la grille EVALADOM

Abstract : Despite France’s population ageing, preventive home visits are neglected. The EVALADOM scorecard assesses the patient his/her own home. Three theses have contributed to prove the internal consistency of the tool, its feasibility of use at primary health care level and its reproducibility. In the present study, the main objective is to demonstrate the pronostic value of this scorecard following incidents such as falls, fires, deaths, institutionalisations, hospitalisations, increase in social aids. The study is a prospective, multicentre, nonrandomized and observational study in the Isère basin. The source population comprises 163 voluntary elderly patients over 75 years, living at home or in a residential home, who were recruited by general practitioners between August and December 2013. Each patient is visited at home in order to conduct the EVALADOM scorecard as well as tests of the standardised geriatric assessment. A telephone follow-up is undertaken within 3 months to look for the occurrence of events, primary endpoint. 153 patients are evaluated at home. 69.3% are women with an average age of 83. 98 events have been reported. 51 patients present at least one event. The first multivariable analytical model shows a lack of correlation between the overall score of the EVALADOM and the occurrence of adverse events at 3 months. A second model highlights predicative factors of adverse events at 3 months as female sex, subscores ‘nutritional risk’ and ‘patient’. The study will continue with a revaluation of the events at 8 and 12 months. EVALADOM could improve the quality care and screening for elderly patients at home.
