Prise en charge des infections tuberculeuses latentes de l’enfant de moins de quinze ans à La Réunion

Abstract : The fight against Latent Tuberculosis amongst children is a sensitive case on our island. Indeed, Reunion Island, a French overseas territory, with a low incidence of Tuberculosis, is under permanent vigilance by the Anti tuberculosis Center (CLAT) because of a fragile population, especially in pediatrics.And furthermore because of its geographical location within the Archipelagos of Mascareignes, a zone with a high incidence Tuberculosis. This thesis is a qualitative and descriptive investigation that evaluates the activity of CLAT about the management of Latent Tuberculosis (ITL) within the children of Reunion Island. The investigation aims to provide a summary and a comparison of the practice of the different CLAT especially before the changes introduced by the french guidances of october 2013. The study points out harmonious practice between CLAT but also compare to the french guidelines about management of pediatric ITL.The goal of Public Health is very well carried out. Disparities exist essentially within the number of investigation conducted by the different Center due to different interpretation of contagious circles. The outcome of this study shall provide a decision tree to manage children Latent Tuberculosis in Reunion Island that intertwines the new medical guidelines of 2013 without forgetting the particularities of Reunion Island.This decision tree propose to use both methods of screening , TST and QuantiFERON, in order that clinician obtain a clearer indicator of risk of developing tuberculosis infection especially among a fragile pediactric population. This work aims to establish a basis for discussion on the coordination of Anti Tuberculosis Center in futur.
