Reconstruction vaginale dans les exentérations pelviennes : quelle technique ? Quels résultats ?

Abstract : Pelvic exenteration is a long and mutilating surgical procedure in which one or two permanent ostomies are built and vagina often abducted. Despite the significant physical and psychological complications, this surgery has a place in the treatment of women with cancer in the pelvic area. Improved surgical and anesthetic techniques allows survival at 5 years from 40 to 60%. Vaginal Reconstruction in this context has many advantages. It provides a better post operative scarring, pelvic exams interesting in the context of long-term monitoring; it helps to restore body image and possibly allow a resumption of sexual intercourse. The reference technique is the musculo-cutaneous flap of the rectus abdomen. However, the functional long-term results are disappointing. Enterocolpoplasties, used for many years in Pediatric Surgery or sexual reorientation surgery seem satisfactory. Moreover, the spread of laparoscopy and robotics will likely improve further these results.
