Œdème maculaire et usage de la céfuroxime en prévention de l’endophtalmie après chirurgie de la cataracte

Abstract : Purpose: To study early macular tolerance with optical coherence tomography (OCT) at day one after cataract surgery with intracameral cefuroxime injection. Setting: Foch Hospital, Suresnes, France. Design: Case series. Material and methods: Prospective study in patients undergoing cataract surgery by phacoemulsification with intracameral cefuroxime 1 mg / 0.1 mL injection, excluding patients with a history of macular pathology. Two measures were performed by Stratus™ time domain OCT preoperatively at day 0 and postoperatively at day 1: centrofoveal macular thickness and macular thickness of the central 1000 microns mapping. We compared these two macular thickness measures at day 0 and day 1 for each surgical procedure. Results: 95 eyes (85 patients, mean age 74.7 ± 9.2 years) were included. The mean centrofoveal macular thickness at day 0 and day 1 was respectively 184.6 ± 31 μm and 185.3 ± 34 μm. The mean macular thickness in the central 1000 μm at day 0 and day 1 was respectively 213.9 ± 27 μm and 215.3 ± 27 μm. There was no significant difference between centrofoveal macular thickness at day 0 and day 1 (p = 0.773), nor between central 1000 μm macular thickness at day 0 and day 1 (p = 0.549). The macular profile in OCT at day 1 was normal without any detected anatomical modification. Conclusions: Early macular tolerance at day 1 after intracameral cefuroxime 1 mg / 0.1 mL injection at the end of cataract surgery is good, with no increase of the macular thickness in OCT. When macular edema appears at day 1, it is necessary to discuss a possible overdose of cefuroxime as a possible etiology.
