Évaluation des non administrations des médicaments radiopharmaceutiques au CHU de Grenoble : bilan, impacts économiques, causes et mise en place d’un plan d’actions

Abstract : Radiopharmaceutical drugs (RD) have specificity to contain short term period radionuclides. Therefore they have got a limited time of use which could cause a financial loss in case of non-administration (NA) to patient. A study has been conducted at CHU Grenoble to value the economic impact and also to seem for the causes for the period 2012-2013. The results confirmed that 239 ( 2%) out of 10980 of Radiophaceuticals have not been administrated. Most of the NA is of the extemporaneous preparations (73%) while specialties concern only 23% of all NA. The total cost of NA is valued to 27000 euros with 20000 euros of only specialties, especially 18FDG. This NA is mainly due to last minute appointment cancelation with more than half of patient truancy (59%), 16% are due to health issues and 12% to contraindication for the test. NA rate of RD is relatively low but the cost stays considerable. To better follow the causes of those late cancelations, we have been working on a new way of structuring the software GERA to allow an automatic ceasing of pre-defined causes. The anticipated pharmaceutical validation allows reducing most of the cost effective RD NA. At the end the automatics and customized short message system (sms) could be used, reminding the patient of his appointment.
