L’électrocardiogramme dans la visite de non contre-indication à la pratique sportive en compétition entre 12 et 35 ans. Enquête d’opinion auprès de médecins du sport

Abstract : Background: To improve screening for cardiovascular diseases at risk of sudden death, the French Society of Cardiology (FSC) recommend that pre-participation cardiovascular screening for young competitive athletes between 12 and 35 years old includes a resting electrocardiogram (ECG) in addition to questioning and physical examination.Objectives: Evaluate the professional practices of sports physicians in the light of recommendations. Gather their views on this recommendation in their professional practice. Assess barriers to implementation of screening.Method: Survey of 1 197 sports doctors practicing in France, taking the form of a questionnaire sent by mail on 04/12/2013, whose anonymous responses were collected until 11/12/2013.Results: 471 physicians responded to the survey. 78.1% are aware of the SFC's recommendations. 72.2% are equipped with an ECG, 38.6% use it during the pre-participation exam . 52% believe they could implement the SFC's recommendations. The cost of the recommendations and lack of time are the two main obstacles of ECG screening. Lack of training is in third position. 60.5% of physicians surveyed consider themselves proficient in ECG interpretation but only 13.9% of sports doctor had mentioned cardiomyopathy when we asked what pathology could be diagnosed during ECG screening. Conclusion: There are many barriers to the implementation of a screening program by sports doctors. Improved training in ECG interpretation is necessary. This requires adoption of a consensus on positive ECG criteria. It is necessary to improve epidemiological knowledge about sudden death of young athletes, mainly including mandatory reporting of such events.
