Modification des pratiques et des appréhensions lors des gestes techniques chez les étudiants en TCEM 3 de DES de Médecine Générale ayant subi un accident d’exposition au sang

Abstract : Background and purpose: Occupational exposure to blood (OEB) is a daily event of the medical practice. While underreported, the potential consequences are significant, in terms of health risk, psychological impact, and financial cost. The objective of this study was to analyze the impact of OEB on the practice of future GPs. Method: This is an investigation of cross practice with TCEM 3 students of General Practice of Bordeaux. A questionnaire was submitted to each student, gathering informations on previous OEB, declaration, conditions of occurrence, severity of the accident and potential consequences on the technical practice. Main results: Among the 200 students, 155 questionnaires were usable. Of these, 66 students (43%) had one or more OEB, mainly in Emergency departments/ Intensive Care Units (50%), medicine unit (20%). The rate of unreported OEB was 42%. The main reasons of under-report were the lack of gravity or lack of time. The accident was preventable in four cases out of five. The majority of students (64%) think that OEB improved their practice, particularly in terms of knowledge and adherence to universal precautions, of knowledge of behaviour in case of OEB, and communication with younger students about OEB. More than three quarters of students did not report any increasing of apprehension in the technical practice. There was no avoidance strategy. Conclusion: Nearly half of the future general practitioners underwent OEB during training. This leads into an improvement in their adherence to universal precautions. Nearly half of them were not reported. Better knowledge of universal precautions would reduce the prevalence of OEB
