Évaluation de l’impact du délai d’instauration du sulprostone dans la prise en charge de l’hémorragie du post-partum à l’hôpital Trousseau (2012)

Abstract : Postpartum haemorrhage is the leading cause of maternal mortality, in France. When oxytocine appears to be ineffective, sulprostone must be started within 30 minutes of the diagnosis. A retrospective study was conducted in the Trousseau hospital over 70 deliveres in 2012 to determine the maternal complications when the delay between diagnosis and sulprostone administration is prolonged. Every patients who received the preventive treatment have been expelled, after blood loss estimation.There was no significant difference observed between groups concerning surgeries, blood transfusions and transfers in intensive care unit. The correlation between initiation treatment moment and the severity of postpartum haemorrhage wasn’t proved. Prostaglandin treatment failure can be explained by a late diagnosis, a treatment initiation delay or a massive postpartum haemhorrage. A multicenter, randomized study could confirm or refute these results and assess the causes of treatment failures.
