Évaluation des délais de reperfusion des SCA ST+ en Béarn et Soule : étude prospective à partir du registre REANIM en 2012 et 2013

Abstract : Introduction: Assessment of delay between first medical contact and reperfussion therapy in STEMI is needed to verify that real-life daily practice is in keeping with that is recommended in the guideline. Methods: From January 2012 to december 2013, we prospectively evaluated the system delay between the first medical contact and reperfussion therapy in patient with STEMI. This multicentric study from REANIM registry was performed in Béarn and Soule french area. Results: 148 patients were included (75% men, mean age of 65 years). All patients with clinical presentation of STEMI within 12h had reperfusion therapy performed. One fibrinolysis has been administered. The median delay was 70 min out of percutaneous coronary intervention (PCI) hospital. The rate of patient with delay above 90 min is 40%. The median delay in Pau area is 60 min (CI95 57-83min), 113 min (CI95 85-159min) in Orthez area and 133min (CI95 98-178min) in Oloron-Sainte-Marie area. It was noted that geographic origin and duty schedule are statistically related with longer delay. The median delay was 44 min in PCI hospital. Conclusion: Our results suggest that a part of STEMI patient should be treated with fibrinolysis due to their system delay above current recommendations.
