Évaluation par les patients atteints d'hépatite C chronique du rôle du médecin généraliste dans leur parcours de soins

Abstract : Background and Objective: In France there is currently a paradox between more efficient diagnostic and therapeutic methods for hepatitis C and a lack of screening with 43% of HCV patients who do not know their status. Nevertheless, hepatitis C, with the arrival of new antiretrovirals, would be the only chronic disease possible to heal. There is therefore a public health issue to mobilize doctors to a better management of this disease. In this context we wanted to know the role of the general practitioner in the course of patient care. Methods: We examined 46 patients with chronic HCV, followed in Savoie, between March 1 and August 27, 2014. They completed a self-administered questionnaire for the screening, monitoring and management of their disease. Results: Regarding the screening: 73.3% of GPs did not search for risk factors for HCV among their patients. 21.7% of patients discovered their positive status in another circumstance. For monitoring and management: 70.5% of patients were followed jointly by the specialist and the generalist. 30% of patients felt insufficiently informed by their general practitioner mainly about their treatment (40%). The time before the first consultation by the specialist was more than 6 months for 23.3% of patients. The reasons for delay of management were varied, including psychosocial origin. Conclusion: The role of the general practitioner in the course of care for patients with HCV must be optimized to better identify and treat patients. The expected benefit is individual and collective, since by healing a significant number of patients we could reduce the risk of new infections and thus consider eradication of hepatitis C. This challenge deserves greater involvement of physicians and a better pluridisciplinary work between nurses, specialists, GPs.
