Étude de l’efficacité et de la tolérance d’un implant intra-vitréen de dexamethasone (Ozurdex®) dans le traitement de l’œdème maculaire uvéitique : étude rétrospective sur 27 yeux

Abstract : Objective: To study the effectiveness of Ozurdex® on 1mm central average macular thickness (EMM) and on visual acuity in eyes with uveitic macular edema. Materials and Methods: This is an observational retrospective single-center study. The main parameters collected are: automated measurement by optical coherence tomography (SD-OCT) of the EMM, the different characteristics of macular edema in OCT, the best corrected visual acuity and intraocular pressure. Quantitative variables were described in the form of mean, standard deviation and extremes (minimum and maximum). Qualitative variables were described in terms of the numbers and frequencies. Results: 27 eyes of 23 patients were included. After an initial injection, we find a 45% decrease of the EMM until three months, and then a limitation of the efficiency with an EMM reduction of only 21% between 3 and 6 months. Less activity on the EMM is noted after 3 months for a second dose, with only 4% improvement. The action profile on visual acuity is the same as the EMM after a first injection, with 34% improvement before 3 months, 24% after 3 months and 17% at 6 months. Activity in visual acuity seems is slightly prolonged compared to the EMM for a second implantation. Reinjection rates are the same after one or two injections, and the need for reinjection is more than 50% of eyes around 6 months. Conclusion: Ozurdex® seems to be one of the first local treatment combining a notable efficiency on the macular edema with a recognized employment security. Our study confirms the data in the literature in terms of efficiency on uveitic macular edema (for a first injection) and in terms of action duration between 3 and 4 months.
