Exposition paternelle médicamenteuse : étude descriptive à partir de la base de données nationale Terappel

Abstract : Studies show that drugs were prescribed and dispensed to a quarter to a third of fathers before conception and to a great number of them during the pregnancy of their partner. No significant increase in the risk of adverse pregnancy outcome was observed in case of drug paternal exposure. However, few studies have investigated this issue. In the first part, we describe data from the literature about paternal exposure and pregnancy outcome. The potential mechanisms of toxicity that may cause deleterious effects on pregnancy are addressed. Considering the lack of data on this subject, we have performed a descriptive study of pregnancy outcomes recorded in the national database TERAPPEL in case of drug paternal exposures. The objective, method, results and discussion of the present study are described in the second part. We identified 242 questions about paternal drug exposure for which the pregnancy outcome was known. These pregnancies led to 186 live-births (76.9%), 27 voluntary terminations (11.1%), 16 miscarriages (6.6%), 12 medical terminations (5.0%) and 1 intrauterine death (0.4%). Among the 198 prospective questions, 8 malformations have been reported (4.0%). Men were mainly exposed to “antineoplastic and immunomodulating agents”, “anti-infectives for systemic use”, “dermatologicals” and “nervous system” drugs. The drugs mostly consumed by fathers were methotrexate (n=49), ribavirine (n=35), interferons (n=31), azathioprine (n=20), isotretinoïne (n=11) and etanercept (n=10). This study did not show any alarming data on paternal exposure and pregnancy outcome but larger scaled or more specific studies are needed to conclude.
