Les cancers de l'enfant et de l'adolescent à La Réunion et Mayotte, 2005-2011

Abstract : Little is known about cancer epidemiology of children and adolescents in Mayotte and Reunion Island (RI), where sociocultural and economical backgrounds are singularly different from that of metropolitan France (MF). The aim of this study is to describe for the first time a population-based cohort of children and adolescents with cancers in these two French overseas departments. All invasive tumors cases (including benign Central Nervous System [CNS] tumors) diagnosed between 2005 and 2011, younger than 18 years of age and registered in the Reunion Cancer Registry were included. We identified 291 cases: 222 from RI (176 children, 46 adolescents), 69 from Mayotte (60 children, 9 adolescents). Incidence for all cancers in RI children was estimated to 121.6/million; the most frequent tumors (leukemias and CNS tumors) were less frequent than in MF. Cancer incidence among adolescents (153.4/million) was also less than in MF. For Mahori children, incidence for all cancers was 101.6/million, CNS tumors and leukemias were less frequent than in MF whereas incidence of malignant epithelial tumors is singularly higher, which could be explained by the high prevalence of xeroderma pigmentosum. More than a lack of exhaustiveness, underlying epidemiological and genetical factors among these 2 populations may explain the difference of incidence observed for some cancer groups. The overall survival rate at 5 years for RI patients was 77.5% (95%CI=[70.8-82.9]), which is slightly lower than the rate in MF. No difference was observed between the survival curves for RI children and adolescents. Further analyses of survival rates per cancer groups were not possible given that number of identified cases were low. Main relevant epidemiological elements found herein deserve however to be explored with specified studies.
