Apport de la TDM de perfusion dans les facteurs prédictifs d'une crise à la phase aiguë d'un AVC

Abstract : Seizure in the acute phase of stroke is a rare event, whose effect on patient outcome is unclear. Several studies agree on clinical risk factors (stroke severity) or radiological (cortical involvement, hemorrhagic transformation), but none consider the initial CT perfusion. Objective: Determine risk factors of acute phase seizure using CT perfusion. Patients and methods: From eligible patients for intravenous thrombolysis who underwent a perfusion CT and with supratentorial ischemic stroke, we conducted a case-control imaging analysis, comparing CBF, CBV, MTT and TTP, of the ischemic core and penumbra, among a population of patient with an acute seizure. Clinical and biological factors analysis were performed in parallel on the selected population. Results: Data from 279 patients were collected. 15 (5.7 %) had seizure, and 32 are included in the imaging analysis (11 cases and 21 controls). There is no significant perfusion parameter associated with an acute seizure. Total cholesterol is negatively correlated with the acute phase seizure (p=0,04), while hemorrhagic transformation is a risk factor for seizure (p=0,04). Conclusion: No risk factors were highlight, from initial CT perfusion. More patients will be needed to confirm or refute the trend in the CBF.
