L’écologie des soins médicaux étudiée sur une population parisienne en 2013

Abstract : Introduction: The concept of the ecology of medical care first described by White in 1961, represents an important contribution to improving the health system. 53 years later, this concept remains unacknowledged to its full extent and has insufficiently impacted education and research in general practice. The main objective of this study was to visualize the ecology of medical care in 2013 within a Parisian population. The secondary objectives were to verify the White’s law and to assess the triggering factors of consultation and the self‐management strategies of the participants. Methods: An observational, mono-centric, prospective study was taken during the month of October 2013. An agenda was sent to 500 randomised adults from a patient base of a general practitioner working in the 14th arrondissement of Paris. The participants were asked to indicate any perceived symptoms and their way of management. Results: The participation rate accounts for 33.6%. Of these, 81% were symptomatic and 27.9% visited a physician. The results were consistent with White’s findings proving the timelessness of the concept. The White’s law was verified and various triggering factors of consultation which were statistically significant were identified: age > 50 years, absence of professional activities, divorce, rash, joint pain. This study assessed that 70% of the symptomatic participants chose self‐management strategies which were in turn analysed on a qualitative basis. Conclusion: This study provides the analysis of the ecology of medical care with new perspectives. This inventory is witness to the persistent shortcomings of education and research in the field of general practice.
