Faisabilité d’une question brève sur le nombre de partenaires sexuels comme préalable au dépistage en médecine générale de l’infection à Chlamydia trachomatis. Étude Dépichla

Abstract : Background: Chlamydia trachomatis is the most frequent sexually transmitted disease (STD) in developped countries. This infection has become a public health issue considering its potential long term complications (ectopic pregnancies, infertility). This study evaluates a short question on the number of sexual partner(s) in a young female patient population, in order to perform a targeted screening of Chlamydia trachomatis infection in family practice. Methods: general practitioners (GP) asked 20 female patients (age: 18-30 years) about the existence of a new sexual partner in the three past months or of several partners in the previous year. Depending on the answer, multiple data were collected (course of the consultation, STD test prescription). Results: 37 GP included 575 patients. 413 patients (71.8%) answered the short question. This question identified 124 patients (21.5%) with a high risk of STD and targeted screening was performed for 81 of them (14.1%). 64 patients (11.1%) had a combined screening with HIV. 18% patients felt discomfort about the question. GP felt discomfort in 14% of the situations and more significantly with patients under 25 years old (p<0.05). The duration and the orientation of the consultation were modified respectively in 30% and 50% of cases. In 26% of patients, the question led to other sexual problems. Conclusion: systematically asking about the number of sexual partner(s) in female patients between 18 and 30 years old is possible in family practice. This approach allows targeting patients with a high risk of Chlamydia trachomatis asymptomatic infections.
