Influence du surpoids et de l'obésité sur l'induction du travail : étude cas-témoins sur 351 patientes au CHU de Bordeaux entre 2009 et 2011

Abstract : Objective: To estimate the impact of a « Body Mass Index (BMI) ≥ 25kg/m2 » on the delivery mode after induction of labor (IOL) with prostaglandin E2 (PGE2) or ocytocin. Methods: We performed a retrospective monocenter study between 2009 and 2011 at the Bordeaux University Hospital, France. Patients with singleton pregnancy were enrolled whatever their comorbidities, term or indication of IOL ; and were divided into two groups : « case » (BMI ≥ 25kg/m2) and « control » (BMI < 25kg/m2). The choice between PGE2 or ocytocin were Bishop score dependent. Main outcome measures: Rates of caesarean section and indications in the two groups. Results: One hundred and eighteen patients who had a BMI ≥ 25kg/m2 were matched according to the month of delivery with 233 patients with BMI < 25kg/m2. The rate of caesarean section after IOL was 28,8% in the case group against 16,7% in the control group (p = 0,009). After multivariate analysis, the aOR was 2,03[1,03-4,00] (p=0,04). The rate of caesarean for failed induction was 7,6% and 2,1% for the case and control groups, respectively (p = 0,02). Conclusion: After IOL, risk of caesarean section is higher for women with BMI ≥ 25kg/m2; mainly because of the rate of failed induction. This risk factor has to be associated, prior to IOL decision, with the other well known factors in literature which are maternal age>35 years, Bishop score < 3, nulliparity and excessive weight gain during pregnancy.
