La conciliation des traitements médicamenteux en géronto-psychiatrie : quelles sources d’informations utiliser ?

Abstract : This prospective study describes the establishment of the Reconciliation of Medicated Treatment (CTM) in Geriatric Psychiatry at the Centre Hospitalier Charles Perrens. The CTM was executed at the admission and at the discharge of the patient. The collected datas, the clinical impact and the professionals contentment are analyzed. A statistical comparison of the sources of information used (criteria availability, completeness, wholeness and time) highlighted the most reliable ones. At the discharge, a letter sent to the pharmacist and to the physician promotes the link city-hospital. During 7 months, 110 patients (mean age 78 years) were included. The average time to complete the CTM is 62.8 min, overestimated by the systematic examination of all sources. 40% of patients have been reconciled within 24 hours (MR1 index High 5s). There is a low explicitness of the prescription (MR2) with an average of 2.9 Intentional Discrepancies Not Documented. Only 2.72% of patients had no difference (MR5). An average of 2.1 DNI per patient was observed (MR3). 76.4% of patients had at least 1 DNI (MR4). Drug omissions (76%) were mainly detected. The clinical impact is positive in 79% of cases. Concerning the comparison of 12 potential sources of information, we have demonstrated that the patient is uninformative for the development of BMO but, he is essential in a process of clinical pharmacy. Trinomials sources to interview to be the most reliable are "Patient-Pharmacist- Letters" or "Patient-Pharmacist-Nursing homes" according to the place of origin of the patient. Health professionals (city and hospital) are satisfied. A track for improvement is to send faster the document. This study confirms the value of interdisciplinary collaboration and city-hospital as part of securing patient’s care.
