Instauration du journal de bord à titre expérimental en stages ambulatoires à la faculté de Paris Descartes : analyse descriptive au cours du semestre d’hiver 2012-2013

Abstract : Context: The internships artwork during the studies of General Medicine expects one or two six-month internships in ambulatory medicine: Level 1 internship and Ambulatory Internship in Primary Care and Supervised Autonomy. These are essential as they allow construction and skills acquisition that are necessary for residents’ future practice. The adopted process by the General Medicine Department of Paris Descartes involves certification of these skills’ acquisition by evaluating written works during these trainings. A logbook was established during the winter semester of 2012-2013, as a new learning track. It must include the clinical situations experienced by the students which are classified into one of 9 defined items. Objective: evaluate the logbook establishment and optimize its use. Method: quantitative analysis of 53 logbooks and study of 722 cases of item A "unpublished care situations" according to the clinical context, apprenticeship, supervision and bibliography. Results: 53 logbooks analysis showed good interns adhesion but ranking in the 9 items was visible only in 2/3 of logbooks which does not allow the assessment of the situations diversity. Study of 722 cases of item A has highlighted different apprenticeships in 96.2% of cases but neither their progress nor relevance could be assessed. Conclusion: General practitioner trainer plays a key role in overseeing and assisting in logbook redaction. Specific education would allow appropriation of this track and better GP trainer adherence and more accurate evaluation by tutors. It is also essential to involve GP trainer in developing an evaluation grid.
