Aspects cliniques et histopathologiques du mésothéliome pleural malin chez l’enfant et l’adulte jeune : à propos d’une série de 47 cas

Abstract : BACKGROUND: Malignant pleural mesothelioma (MPM) is a rare tumour with a poor prognosis, mainly occurring in adults- asbestos exposed and exceptionally in young patients. Our aim was to evaluate the clinicopathological particularities of MPM in patients (MPMy) i.e. under 40 years. DESIGN: 47 patients (18 women, 29 men, from 13 to 40 years) have been selected from the CNR MESOPATH/MESOBANK database-Caen, France and compared to a control population (CP) of 423 patients (> 60 years), randomly selected from the same database between 1998 and 2013. Clinical data (asbestos/radiation exposure, imaging, clinical symptoms and survival), histological type (Epithelioid, Sarcomatoid, Biphasic or Desmoplastic), and immunohistochemical phenotype were compared. Histologic patterns and cytological features were recorded in young patients. RESULTS: There was a slight female predominance in MPMy (38% vs 25%; p=0.05), who were less frequently asbestos-exposed (58% vs 77%; p=0.01) but more often irradiated for chest cancers (16% vs 6%; p=0.07); they presented less frequently pleural effusion (77% vs 97%; p<0.0001), but more often with pleural nodules (39% vs 15%; p=0.0008) and had a better survival (24 mo vs 14 mo ; p=0.01). However, there was no difference regarding histological subtypes (Epithelioid; 81%, Biphasic; 10-15%, Sarcomatoid; 2-6%, Desmoplastic; 2-3%), and immunophénotypes; 14% MPMy harboured rhabdoid features and 8% a myxoid stroma, rarely reported in MPM. CONCLUSION: MPMy affects more often females, less frequently asbestos -exposed and with a better survival. Rhabdoid features and myxoid stroma are not infrequent. Molecular characterisation of MPMy is ongoing.
