Étude médico-économique de la reconstruction mammaire par lambeau libre de type DIEP à l’hôpital : guide pratique du codage en chirurgie plastique

Abstract : Objective: Since the introduction and application of T2A (Pricing Of Activity), coding has become the main indicator of a service activity. We wanted to demonstrate the importance of training in the basic principles of coding in T2A system on the model of breast reconstruction. Material and Methods : A retrospective study of medical and economic data of 37 patients who underwent breast reconstruction with abdominal DIEP free flap between january 2012 and september 2013 in the department of plastic surgery at Pontoise hospital. We analyzed the revenue stays based on DRG. Results : In the absence of specific act of CCAM DIEP, patients DRG were divided into 2 groups: a "breast" DRG for breast reconstruction and "skin" DRG. Hospitalization of patients in "skin" DRG were less profitable. Our study shows that breast reconstruction with DIEP was not valued optimally due to poor orientation of DRG and an undervaluation of DRG because of complications or comorbidities omission in coding. Conclusion : Knowledge of coding rules can improve the profitability of stays.
